Minnesota Court Forms

The Minnesota Courts website has forms related to filing an order of protection with an option to fill out the forms online, known as ”Fillable Smart Forms.” They also offer court forms related to custody and parenting time, divorce forms, small claims court forms, and forms for many other types of cases on the Minnesota Judicial Branch website. You can also get the forms from the clerk of court when you go to the courthouse. To find the courthouse in your area, go to our MN Courthouse Locations page.

A domestic violence organization in your area may also be able to provide you with the necessary forms or help you to fill them out. You can find one near you on the MN Advocates and Shelters page.
