You can also download forms for the petition and affidavit for a civil protection order due to sexual abuse, sexual assault, or stalking. Court forms are also available in the civil clerk’s office at the courthouse. See CT Courthouse Locations for a list of local courthouses.
A local domestic violence organization or legal aid office may be able to help you find or fill out the forms you need.
